2024-25 Indoor Club Tryout Registration

Club Sand Volleyball Terms of Agreement I acknowledge and agree to the following: 1. Club Sand Volleyball or its affiliates, in addition to the payment of any fee or charge I do hereby forever waive release and discharge Club Sand Volleyball and its officers, agents, employees representatives, executors, and all others acting on their behalf from any and all claims or liabilities for injuries or damages to my person and/or property, including those cases by the negligent act or omission of any activities, programs or services of Club Sand Volleyball. 2. I have been informed of, understand and am aware that any sport involves strength, flexibility, and aerobic exercise, including the use of equipment, is a potentially hazardous activity. I have also been informed of, understand, and am aware that fitness activities involve a risk of injury, including a remote risk of death or serious disability, and that I am voluntarily participating in these activities and using equipment and machinery with full knowledge, understanding and appreciation of the dangers involved. I hereby agree to expressly assume and accept any and all risks of injury or death. 3. I so hereby further declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity or other illness that would prevent my participation or use of equipment or machinery. I understand that it is recommended that I have yearly or more frequent physical examinations and consult with my physician about physical activity, exercise and use of exercise equipment. 4. I understand that Club Sand Volleyball’s provision and maintenance of all exercise/fitness program for me does not constitute and acknowledgement representation or indication of my physiological well-being, or a medical opinion relating thereto. Photography Consent Form/Release: You (the parent or guardian of the registered participant) hereby grant permission to Club Sand Volleyball representatives,to take and use: photographs, videos and/or digital images of my child for use in news releases and/or promotional materials. these materials might include printed or electronic publications, web sites, or other electronic communications. I further agree that my child’s name and identity may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation to me. All negatives, prints, digital reproductions shall be the property of Club Sand Volleyball representatives. or other program held at Club Sand Volleyball. Financial Agreement I understand that it is my responsibility to cover all fees as stated in the program’s registration regardless of whether my player finishes the season (Medical Release Exception). In the event the account is delinquent and satisfactory arrangements have not been made for payment, the undersigned agrees to pay a $30.00 late fee on all past due invoices if not paid by the 5th of each month. Additionally, I understand that my child will not be allowed to be placed on a team if there are any outstanding balances from previous season. There will be a $50.00 penalty for any credit card charge back request (issued through your credit card company) and any approved refunds are to be processed through Club Sand Volleyball’s administration. NON-REFUNDABLE deposit due no later than one week prior to the uniform deadline.
